Welcome to our Transition Applicant Portal! This portal is designed for employees who are currently working at a facility that has entered into an agreement to transition healthcare services to VitalCore Health Strategies and wish to retain employment.
To initiate the transition onboarding process please select the location you currently are working at below which will take you to a password protected page. If you have not been given a password, please send a message to the email address listed on the welcome letter for your site location that can be found under the “Announcements” page of the VitalCore website.
Once you have entered the password, select the link to the Onboarding Career Center where you will be able to apply for your “current position” as you will need to complete the onboarding process before initiating a change in role or location.
It is imperative that all employees interested in retaining employment during a transition of services complete this process. Failure to complete this process will result in VitalCore assuming that you are not interested in continuing employment and pursuing other applicants to fill your position.

Note: Our initial goal during a transition is to retain employees in their current positions. If you are interested in changing positions and/or locations, you can request/apply once your current position is secured, and we have completed the onboarding process for the transition.
